Residential Door Rebate Application

All sections must be filled out, signed, and dated by the building owner. An incomplete form will not be processed. Submit a completed form with the following documentation; a receipt or contractor invoice, before and after photos, and proof of Energy Star certification (examples: door sticker, product qualification label, screenshot of retailer/manufacturer website or product description). You must have properly installed the qualifying door in an existing dwelling unit with a primary electrical heat source to qualify for this program. Programs are subject to change or cancellation without notice. Application must be submitted within 12 months of project completion.
